- Passport valid for at least six months, with blank pages
- Completed visa application form and recent photographs
- Medical invitation or appointment letter from Bangkok Hospital
- Recent medical reports supporting the need for treatment
- Bank statement or proof of funds
- Flight booking and accommodation details
For attendants
- Passport and application form
- Their name included in the hospital letter
- Proof of relationship to the patient where requested
